Through our Client Support Navigators, Community Resources – Rexdale provides clients with useful information and advice about many of the social determinants of health, such as housing, immigration, social assistance, education, and links to employment agencies. We also assist clients with hospital support, referrals, and supportive counseling. We provide assistance in preparing documents and can obtain translation and interpretation services in multiple languages as required.
The Community Resources – Rexdale supports clients through one-on-one assistance or group programs. In partnership with local agencies and community groups, the Community Health Worker at Community Resources – Rexdale can provide health education, support groups, and interpretations.
