Rexdale CHC offers family practice medical services with healthcare providers who work together in teams to improve the health and well-being of our clients. Our family healthcare team is open to new clients, and our services are available to residents who live in the Rexdale CHC catchment area (visitors to Canada are not eligible). Our clients benefit from multidisciplinary healthcare teams that include family physicians, nurse practitioners, and registered nurses. Our services include health promotion, disease/illness prevention and management, as well as rehabilitation services.
Our primary healthcare team offers the following:
One-on-one appointments for individual clients.
Group services are centred around specific themes and objectives for clients and community members (pre-registration is usually required).
Special health education activities for clients and community members (pre-registration may be required).
Pregnancy care for women is available for up to 27 weeks of pregnancy.
Immunization for babies and young children.
Language interpretation services are available for your appointments.
